Youth Football League 2020 Registration Requirements

For New Kaya FC Academy and YFL Players

Requirements:

1. 2 pieces of 2×2 Passport Photos (Must taken professionally or from studios.  No Facebook Pictures, No Cropped Photos)
2. Coloured copy of birth certificate or the 1st page of the participant’s passport.
3. Completed YFL Registration Form. Click HERE to download.

All requirements must be completed and must be in hard copy. (No soft copies via email/Viber or Facebook)

Failure to comply will lead to delayed participation in the YFL or worse, potential ineligibility to compete in the tournament.

For New Kaya FC Academy Players but Previously Played for a Different Team Last Season

Requirements:

1. 2 pieces of 2×2 Passport Photos (Must taken professionally or from studios.  No Facebook Pictures, No Cropped Photos)
2. A completed, hard copy of the YFL Drop Form signed by the previous club. Click HERE to download.
3. Completed YFL ID Renewal Form.  Click HERE to download.
4. Old YFL ID

All requirements must be completed and must be in hard copy. (No soft copies via email/Facebook/Viber)

Failure to comply will lead to delayed participation in the YFL or worse, potential ineligibility to compete in the tournament.

For Re-Registering Kaya FC Academy YFL Players

Requirements:

1. 2 pieces of 2×2 Passport Photos (Must taken professionally or from studios.  No Facebook Pictures, No Cropped Photos)
2. Completed YFL ID Renewal Form. Click HERE to download.
3. Old YFL ID

All requirements must be completed and must be in hard copy. (No soft copies via email/Viber or Facebook)

Lost IDs

Requirements:

1. A notarized affidavit of loss
2. ID Replacement Fee

Please take note in your renewal form if you have lost your ID.
